National Family Caregivers Month Each November, National Family Caregivers Month honors the millions of Americans who provide care and support for loved ones living with chronic illnesses, disabilities, or age-related conditions. GUIDE is a covered Medicare service with no cost to eligible patients. Overall, this is a phenomenal program that helps folks function longer in the home environment.”

It recognizes that unpaid caregivers, often family members, are the foundation of dementia care, and seeks to ease their burden through structured support, education, and coordination.

Under the GUIDE Model, participating organizations like First Choice Neurology provide a dedicated care team that helps families navigate the complex medical, emotional, and logistical aspects of dementia care. This includes:
Comprehensive Care Planning: A personalized approach that aligns medical treatment, home care, and community resources with the patient’s evolving needs.
Caregiver Education and Support: Dementia-specific training, stress management resources, and continuous access to your care team for questions or concerns.
24/7 Access to Support: Continuous communication with a care navigator or team member who can assist with urgent needs or provide emotional reassurance.
Respite Services: Programs that give caregivers temporary relief, allowing them time to rest and take care of their own well-being.
